GENERAL TERMS AND CONDITIONS FOR THE WOMAN’S PACKAGE GIVE-AWAY

1. Eligibility

1.1 You must be at least 18 years of age by March 31, 2010 to enter to win The Total Woman’s Package Give-Away. 


1.2 You must be a resident of Okaloosa, Walton or Santa Rosa County as of March 31, 2010, to win The Total Woman’s Package Give-Away. 


1.3 Employees of White-Wilson Medical Center and their immediate families (spouses, parents, children, and siblings and their respective spouses) are not eligible to win. Persons affiliated with remote offices for White-Wilson Medical Center, affiliates, distributors, advertising and promotion agencies or related organizations and their immediate families are not eligible to win. 


1.4 Both men and women are eligible to enter; however, only women are eligible to redeem the screening mammogram and bone density scan at White-Wilson Medical Center. Additionally, it is recommended that participants be of 40 years of age, as the screenings provided in the prize are geared towards individuals ages 40 and over. 



2. Timing
Participants can enter to win The Total Woman’s Package Give-Away between 12:00 AM CST on September 1, 2009, and 11:59 PM CST on March 31, 2010. 



3. How to Enter

3.1 Participants can enter to win at: http://www.white-wilson.com/littleblackdressparty_register.htm


3.2 Participants must include their correct name, address, city, state, zip code, phone number, email and primary care physician to win. Entries made with incorrect data are subject to be void at the sole discretion of White-Wilson Medical Center. 


3.3 Multiple sign-ups or entries will not increase your chances to win. 




4. Prizes

4.1 There are ten Total Woman’s Packages to be given away. The packages include a certificate for a screening mammogram, coronary artery calcium scoring and a bone density scan from the White-Wilson Medical Center Department of Imaging. Additional prizes may be included at the sole discretion of White-Wilson Medical Center. 


4.2 Certificates must be redeemed at the White-Wilson Medical Center Department of Imaging, located at 1005 Mar-Walt Drive, Fort Walton Beach, Florida. The certificates for the screening mammogram, coronary artery calcium scoring and a bone density scan are only valid at White-Wilson Medical Center. 


4.3 Winners are fully responsible for any additional expenses that may or may not be incurred as a result of test results, including but not limited to additional exams, physician appointments, resulting medical or hospital fees, and so forth. 


4.4 The mammogram, coronary artery calcium scoring and a bone density scan certificates in the prize package expire December 31, 2010, without exception. 


4.5 The winner is responsible for scheduling an appointment to redeem the certificates for the screening mammogram, coronary artery calcium scoring and a bone density scan before they expire on December 31, 2010, and must present the certificate at time of payment. 


4.6 Prize details are subject to change, in which case a prize of equal or greater value will be substituted at the sole discretion of White-Wilson Medical Center. 



5. Winner Selection

5.1 Winners will be randomly drawn before the Little Black Dress Party set for March 31, 2010. White-Wilson Medical Center is subject to change the date of the drawing without notifying the participants. The potential Prize Winner must meet all eligibility requirements set forth in these rules to qualify for prize.


5.2 Winners are responsible for redeeming the package within 60 days of notification. Winners will be announced at The Little Black Dress Party in 2010, and will be contacted directly via email and/or telephone. If the winner(s) does not accept the prize via email, mail or telephone within 60 days then a new winner will be randomly drawn. 


5.3 Men are not eligible to redeem a bone density scan or a screening mammogram at White-Wilson Medical Center. Men are permitted to transfer the certificates for a screening mammogram and/or bone density scan to a woman so long as she lives in Okaloosa, Walton or Santa Rosa County and is over the age of 18 as of March 31,2010.


5.4 White-Wilson Medical Center recommends that individuals that receive a coronary artery calcium scoring are 50 years or older. Additionally, White-Wilson Medical Center recommends that women that receive a mammogram screening or a bone density scan are at least 40 years in age. Individuals that receive one of the above mentioned screenings before the recommended ages do so at their own risk, and may be subject to unnecessary radiation. Participants release and agree to hold White-Wilson Medical Center and its staff and physicians harmless for any liabilities. 



6. General Conditions

6.1 NO PURCHASE NECESSARY. A PURCHASE DOES NOT INCREASE YOUR CHANCES OF WINNING. 


6.2 You do not need to be a patient at White-Wilson Medical Center to be eligible to win. Being a White-Wilson Medical Center patient does not increase your chances of winning. 


6.3 Prize is non-transferable, with no cash redemptions or substitutions except at the sole discretion of White-Wilson Medical Center. 


6.4 Acceptance of prize also constitutes permission to use winner's name, biographical information, and/or likeness for Sponsor's promotional and/or advertising purposes without notice or compensation. By participating, entrants agree to be bound by all the Official Rules of this Give-Away. 


6.5 Participants release and agree to hold harmless White-Wilson Medical Center, and its share holders, affiliates, and subsidiaries or its respective officers, directors, employees, physicians, representatives and agents from any and all liability for any loss, harm, damages, costs or expenses, including without limitation, property damages, personal injury and/or death, arising out of participating in the promotion, the acceptance, possession, use or misuse of any prize, or claims based on publicity rights, defamation, invasion of privacy, or merchandise delivery, participation in this Give-Away, or participation in any of this Give-Away's related activity. 


6.6 Additionally, potential Winner's release and agree to hold harmless White-Wilson Medical Center, and its share holders, affiliates, and subsidiaries or its respective officers, directors, employees, representatives and agents from any and all noncompliance with the terms of these Official Rules and/or federal, national, state, provincial, municipal or local laws or regulations. 


6.7 Internet entries will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the e-mail address by an Internet access provider, online service provider, or other organization that is responsible for assigning e-mail addresses or the domain associated with the submitted e-mail address. 


6.8 White-Wilson Medical Center is not responsible for any: (a) telephone, computer, hardware, software, programming or interrupted or unavailable network, server or other connections, network malfunction, or other technical failures or errors, or failure to properly process entries; (b) communication disruption or other forces beyond the reasonable control of the White-Wilson Medical Center, including inability to access the website, submit entries or other disruption related to Internet traffic, virus, bug, or non-authorized intervention; or (c) damage caused by computer virus or otherwise resulting to any computer from entrant's access of the Give-Away website, whether human, mechanical, or electronic. If such malfunction, error, disruption, or damage occurs, and impairs the administration, security, fairness, integrity, or proper play of the Give-Away, White-Wilson Medical Center, in its sole discretion, may suspend or terminate the Give-Away by posting a notice at www.white-wilson.com. If the Give-Away is terminated before the end date, White-Wilson Medical Center will determine the winner from all eligible entries received as of the termination date. Persons found tampering with or abusing any aspect of this Give-Away, or whom White-Wilson Medical Center believes to be causing malfunction, error, disruption, or damage will be disqualified. 


6.9 White-Wilson Medical Center reserves the right to void all entries made through any robotic, automatic, mechanical, programmed, or similar entry duplication method and to disqualify any individual using such a method. By entering this contest, entrants agree that White-Wilson Medical Center has the sole right to decide all matters and disputes arising from the contest and their determination of the winner are final. All federal, state, provincial, municipal and local laws apply. Void where prohibited or restricted by law. 


6. 10 White-Wilson Medical Center is not responsible or liable in any way for any medical issues, existing or new, that may be discovered during a screening mammogram, coronary artery calcium scoring and a bone density scan. White-Wilson Medical Center is not responsible to provide any additional health related services to the winner. 


6.11 White-Wilson Medical Center reserves the right to amend these terms and conditions at any time without prior notice. 


6.12 The obligations of White-Wilson Medical Center shall be suspended to the extent and for the period that performance is prevented by any cause beyond its reasonable control, including but not limited to, acts of God, laws, regulations, orders or proclamations of any governmental entities, judgments or orders of any court of law, acts of war whether declared or undeclared, riot, civil strife, labor disputes, explosions, fires, earthquakes, volcanic eruptions or other natural calamities.
