| White-Wilson offers a records release to patients in order to provide continuous care or for personal usage. We do not charge for continuous care; however, there is a charge for personal use of $1 for each of the first 5 pages and 25 cents for each additional page thereafter. In order to release protected health information, you will need to fax this release back to our office at 850.863.3828. Please allow us 7 to 14 working business days to release your records. If you need to expedite this process, call the number on the form and we will do our best to accommodate you. Please make sure that this form is filled out correctly. We value your privacy and want to ensure that the information is released properly. If you have been referred to another physician by one of our providers, a records release is not necessary.
